2 managers suspended

Hi everyone.I read today that two managers have been suspended for overseeing a hospital system whereby if urgent cancer patients miss their first appointment they are automatically discharged.Subsequently many ill patients have not received the treatment they should have and one patient may have died as a result.This practise is obviously unsafe and unreasonable but it made me wonder,.does anyone know if this is a widespread method used when physical illness is involved or peculiar to that hospital.The reason I ask is because with both sets of mental health treatments I've received in two different departments I was told by letter that if I failed yo turn up for the first appointment I would be discharged from the service also.This seems quite harsh when you consider that many people may be scared to go the first time.I would have thought that they should at least ring to confirm that the person concerned in fact does require to be discharged.It is possible that this practise could also result in someones death eventually.
